Data collected will be stored on secure Trust systems and used for the purposes of performance of a task carried out in the public interest in which home-care products and other products are delivered to patient's homes.
PPS (hosted by Whittington Health NHS Trust) is the data controller and will process your data in accordance with the regulations that apply to:
• Article 6(1)(e) of The General Data Protection Regulation 2016 (GDPR) /Data Protection Act 2018 (DPA18)
In some cases we may be required to process your health data. This is defined as “special category data” by the GDPR/DPA18 and will be used for the purposes of preventive or occupational medicine, medical diagnosis, the provision of health or social care or treatment and the management of health or social care systems and services.

Data may be shared with our system providers and suppliers in order to provide this service and will be stored on servers based in the UK and will not be transferred outside the EU.
Records will be retained as per the guidance set out in the Records Code of Practice for Health and Social Care 2016.
